My Health My Data Privacy Policy

This notice supplements the Privacy Policy of Glow Medispa, LLC dba Mint Med (“Mint Med,” “we,” “our,” or “us”) and applies to personal data defined as “consumer health data” under the Washington State My Health My Data Act (MHMD).

Consumer Health Data We Collect

The consumer health data we collect depends on your interactions with Mint Med, your privacy settings, the Services you use, your location, and applicable law. Because "consumer health data" is broadly defined under MHMD, many of the data categories we collect may be classified as such.

Examples include:

• Health-related information: Conditions, symptoms, test results, medications, treatments, diagnoses, and clinical status. For instance, if you request a metabolic assessment or prescription services, we may collect this data to facilitate care.

• Precise location information: Data that could reveal your attempt to seek healthcare services. If you consent to location tracking (e.g., via analytics tools), we may collect this to help match you with a provider licensed in your state.

We do not collect sensitive health data using cookies without your prior consent.

As required under MHMD, you have rights regarding your consumer health data, including:

• Right to withdraw consent to collection or sharing of your consumer health data at any time

• Your full name

• The email address associated with your account

• The nature of your request (e.g., access, deletion, correction, withdrawal of consent)

We will respond within the timeframes required under Washington law.

Appeals

If we deny your request under MHMD, you may submit an appeal by emailing help@mintmed.com with the subject line “MHMD Appeal” and include the details of your original request and the denial response.

Complaints

If you feel that your privacy has been violated and need to report a complaint please notify Glow Medspa or the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filling a compliant.
