Consent to Telehealth Services

Introduction

This Consent to Telehealth Services outlines the terms and conditions of using the telehealth services ("Services") provided by Glow Medispa, LLC doing business as Mint Med ("Mint Med"), through our websites and applications. By using these Services, you may obtain an online consultation with an independent, licensed healthcare professional ("Provider") and receive mail-order pharmacy services for any prescribed medications. Telehealth involves the delivery of healthcare services using electronic communications between a healthcare provider and a patient who are not in the same physical location.

Section 1: Telehealth Services

Telehealth services may include, but are not limited to:

• Electronic transmission of medical records, photo images, personal health information, or other data between a patient and a Provider

• Interactions between a patient and a Provider via audio, video, and/or asynchronous data communications, such as secure messaging and email

• Use of data from remote monitoring devices, medical devices, and sound or video files

Section 2: Benefits of Telehealth

• Easier, more efficient, and less expensive access to medical care

• Receiving care and treatment at convenient times

• Access to licensed Providers without requiring in-office appointments

Section 3: Limitations of Telehealth

• Telehealth may provide insufficient information for appropriate medical decision-making, requiring in-person evaluation

• Inability to conduct certain physical assessments may limit diagnostic accuracy

• Potential delays in care due to technological issues

• Possibility of data security breaches despite reasonable safeguards

• Regulatory restrictions may limit available treatment options or prescriptions

Section 4: Patient Responsibilities

By agreeing to this Consent to Telehealth Services, you acknowledge and understand that:

• You have read and understand the risks and benefits of telehealth

• You give informed consent to receive medical care and treatment by telehealth from Mint Med's affiliated Providers

• The technology used in telehealth consultations may include methods not specifically described in this document

• No specific outcomes can be guaranteed; your condition may improve, remain the same, or worsen

• You must provide honest and complete health information, and keep Providers updated on any changes

• Providing inaccurate or incomplete information may result in serious harm or death

• A Provider may determine that your case is not appropriate for telehealth and refer you for in-person care

• Telehealth services support, but do not replace, your existing healthcare relationships

• In a medical emergency, you must call 9-1-1 immediately

Section 5: Privacy and Data Security

• Your personal health information will be handled in accordance with Mint Med’s Privacy Policy

• You have the right to access your telehealth-related records in compliance with applicable laws

• Health records will only be shared with other healthcare providers with your consent

Section 6: Consent Withdrawal

• You may withdraw your consent to telehealth services at any time by emailing us with your request help@mintmed.com

• Otherwise, this consent is deemed renewed with each new telehealth consultation with a Provider
